BBL or a Brazilian butt lift is a popular procedure in body contouring because it removes stubborn fat from areas that patients do not like and uses this same fat to create the curves that patients are looking for.  In one procedure, two areas can be improved at the same time.  FSPS surgeons know that every BBL is different, so they take the time to really understand your preferences to create the curves and overall silhouette that you are looking for. What is a BBL?

A BBL is a powerful technique where the surgeon first uses tiny well-hidden incisions to target stubborn fat and remove it effectively. The surgeon then uses this same fat to create curves and improve contour to your liking.  While some small-volume BBLs can be performed under local anesthesia, usually general anesthesia is used for most BBLs.  A BBL can be performed by itself or in combination with other procedures, such as a tummy tuck when abdominal muscle tightening is necessary. A BBL should always be performed in an accredited surgical facility to ensure patient safety.

Of note, skin is not removed in the upper buttock or lower back area during a BBL, as it would be in a lower body lift.  Your surgeon will explain the differences between these procedures.

Who Is an Ideal Candidate?

The best candidates are individuals in good overall health who have excess stubborn fat and do not have loose skin over the areas where the fat is removed. If loose skin is also present, an additional procedure to treat the skin, such as a tummy tuck or other types of lifts, may be necessary. 

Like all body contouring procedures, you should be at a stable, maintainable weight before liposuction.  You should also avoid smoking, vaping and nicotine prior to surgery.

The Procedure Process

Initial Consultation

Your surgeon will evaluate your anatomy, review your goals, and explain the options that are best for you.  He or she will explain the details of the planned operation including risks, benefits, and alternatives and how you can work together to limit your risks and achieve or surpass your desired results.

Day of Surgery

The procedure is typically performed under general anesthesia. Tiny incisions the size of a pencil eraser are placed in the wrinkles and creases of your body to make sure they are generally not noticeable after the procedure, even when wearing a bikini or a swim suit.

Immediate Aftercare

You’ll be monitored in a recovery area and return home with compression garments and detailed aftercare instructions.  Sometimes your surgeon may prefer an over-night stay.

Follow-Up Visits

Your Board Certified FSPS surgeon will guide your healing process and carefully monitor your progress with scheduled follow-ups.  Post-operative care is an important part of recovery and getting your best result.

Recovery and Aftercare

Initial downtime ranges from 2-3 days, with light activity encouraged as you heal. Most patients return to normal routines within 1 week. Swelling will gradually subside, and results continue to refine for several months. Wearing compression garments and following your surgeon’s instructions are key to optimal healing.

Results: What to Expect

Patients typically notice a significant improvement in contour shortly after surgery. Final results become more visible over 3–6 months as swelling fully resolves. The desired outcome is a natural, enhanced silhouette with long-lasting benefits.

Risks and Considerations

As with any surgery, liposuction with BBL carries some risks, including bleeding, infection, asymmetry, or fluid accumulation.  In Florida, the Board of Medicine now requires placement of grafted fat to be monitored during injection with the aid of ultrasound visualization of the areas of the buttock being treated in most OR facilities.  This ensures the fat is placed accurately in the subcutaneous plane, and not into the gluteal muscles, for a higher degree of safety.  Of note, this safety advance was developed and championed by FSPS members.

Most patients take 3-7 days off from work, and resume light daily activities within 1-2 weeks.  Your surgeon will probably recommend avoiding placing any pressure on the freshly grafted areas, to help the fat survive in the new area.  Depending on the technique used, vigorous activity and strenuous work-outs may require 6-8 weeks of healing.

Yes. While some of the transferred fat will be re-absorbed, about half of the injected fat is “yours to keep” long term, as long as you maintain a stable weight. Future pregnancies or large weight fluctuations can affect your long-term results.
