Breast reduction is a very common procedure for women.  It can provide immediate relief and very satisfying results for women bothered by overly large, heavy and droopy breasts, also known as “macromastia.”  Typically, women can experience significant neck and back pain due to the weight and position of the breastsMacromastia can also limit exercise and certainly affect self esteem.

Breast size can certainly be affected by body weight, but women at an ideal weight can have an overly large breast size, which may be a familial tendency. Women as young as the late teens can safely undergo the procedure, preferably after they are fully grown. Many women proceed after having had their children or even later in life and often wonder why they didn’t proceed earlier. Breast reduction is a transformative procedure performed by FSPS member surgeons using the latest techniques and a personalized approach. What is Breast Reduction?

Breast reduction removes excess breast tissue, fat and skin from the breasts and lifts the breasts creating a smaller, “perkier”, more shapely appearance. It reduces the weight of the breasts.  The surgery can be performed with IV sedation (“twilight anesthesia”) or general anesthesia and typically takes two to four hours.  While techniques vary by surgeon and patient, the typical incision pattern is in the shape of  an anchor or “upside down T” , and is well hidden in a bra or swim suit.  Sometimes liposuction is performed at the same time to improve the contour of the outside of the breasts and chest.   We recommend that the procedure should  be performed under the care of an anesthesiologist or CRNA in a an accredited surgical facility.

The breast reduction is different than a breast lift (“mastopexy”).  In a breast reduction, both the excess breast tissue and skin are removed.  In a breast lift, the skin is tightened and the nipple position is raised, but little breast tissue is removed, leaving the breast size similar to what it was pre-operatively.  Since the breast lift does not reduce the weight of the breast, it doesn’t typically give the symptomatic relief that a breast reduction does.

A breast implant is not typically used during a breast reduction.

Who Is an Ideal Candidate?

The best candidates are individuals in good overall health who have overly large breasts that are causing symptoms and body confidence issues.  Ideally, you should be at a stable, maintainable weight.  You should avoid smoking, vaping and nicotine for several months prior to surgery.

You should be at least 16-18 years old and fully grown. If you are planning on pregnancies and totally committed to breast feeding you may want to consider delaying the procedure until after child bearing.  Although many women are still able to nurse after this procedure, it can not be guaranteed.  Breast imaging with mammogram and/or ultrasound is indicated preoperatively for women age 40, or earlier with a positive family history of  breast cancer.  A consultation with an FSPS member surgeon is your best first step to understanding whether breast reduction is right for you.

The Procedure Process

Initial Consultation

Your surgeon will evaluate your anatomy, review your goals, and explain the options that are best for you.  He or she will explain the details of the planned operation including risks, benefits, and alternatives and how you can work together to reduce your risks and achieve your desired results.  Think about what size you would like to be after surgery, and communicate that to your surgeon.  Typically, this is in the C to D cup range.

Day of Surgery

You will be fasting to prepare for the anesthesia.  Markings are made to plan the incisions.  An IV will be started, so you can receive antibiotics, fluids and other medications.

Immediate Aftercare

You’ll be monitored in a recovery area and return home with a soft compression bra and detailed aftercare instructions.  An overnight stay is not typically required.  Drains may be used.

Follow-Up Visits

Your Board Certified FSPS surgeon will carefully monitor your healing process and check on your progress with scheduled follow-ups.  Post-operative care is an important part of recovery and getting your best result.

Recovery and Aftercare

Initial downtime ranges from 1–2 weeks, with light activity encouraged as you heal. Most patients return to normal routines after 4–6 weeks.  Chest-intensive exercises, like golf and tennis, may take a little longer.  Swelling will gradually subside, and results continue to refine for several months.  Following your surgeon’s instructions is key to optimal healing.

Results: What to Expect

Patients typically notice a significant improvement in their breast size and shape immediately after surgery. Back and neck pain symptoms, as well as posture, can be better right away.

Final results with fading of scars become more visible over 6 -12 months.  The desired outcome are smaller, more proportional and uplifted breasts with long-lasting relief of symptoms.

“Scarless breast reduction”

In women with a mainly fatty breast, typically after menopause, liposuction through 1 or 2 small incisions can be used on the breast itself, in order to reduce the size.  While this technique avoids scarring, it is not always suitable for younger patients, or those with dense breast tissue.  It does not provide the lift that a traditional breast reduction does.

Risks and Considerations

As with any surgery, breast reduction carries some risks, including bleeding, infection, adverse scarring, asymmetry, or fluid accumulation.
